COORDINATION
What is it?
Service Coordination, serving more than 400 individuals and families annually, assists persons with developmental disabilities and their families in building lives of their own choosing. UCP Service Coordinators provide information, linkage and referral to needed services. They also offer advocacy and support for the purpose of empowering people to achieve their personal goals, attain increased independence, and enjoy greater inclusion in the community. The New York State Department of Health, Medicaid, and a grant from the Family Support Services unit of the Long Island Developmental Disabilities Services Office funds these services. Some examples of service assistance areas are housing, transportation, medical care, health benefits, and assistive technology.

Who are the Service Coordinators?
Sometimes referred to as Case Managers, Service Coordinators are experienced professionals who utilize person-centered planning approaches, which focus on the individual’s needs, preferences, and choices. Bilingual services are available.
