DRIVING DIRECTIONS

BY AUTOMOBILE:

Take the Sagtikos/Sunken Meadow Parkway, which is

Exit 41N off the Southern State Parkway OR
Exit 45N off the Northern State Parkway OR
Exit 53N off the Long Island Expressway

to Exit SM3A - which is Indian Head Road. Make a left turn at the end of the exit ramp.

Proceed north on Indian Head Road to the Diagnostic & Treatment Center (on the right side of the road, less than one mile). Please use north entrance.

The Diagnostic & Treatment Center is in the rear building (red), on the Indian Head Campus of United Cerebral Palsy of Greater Suffolk.

BY BUS TO INDIAN HEAD ROAD:

Suffolk Transit Route #56 Bus services Indian Head Road. Connections can be made to the L.I.R.R. Kings Park Station, and to other bus routes in Commack (in the Macy's Shopping Ctr.) and at the Smithhaven Mall.
