FAMILY SUPPORT SERVICES


Sometimes called Case Management, this is a process of linking, coordinating, and monitoring services that are planned by the individual, his/her family and the service coordinator. Experienced professionals utilize person-centered planning, focusing on the individual’s own strengths, needs, preferences, and choices. Some of the services they help obtain and coordinate for more than 250 individuals with disabilities annually include day programs, residential services, medical care, Social Security benefits, employment services, environmental modifications, transportation, in-home services and social/recreational programs.

Home and Community Support Services

Home and Community Support provides a linkage for coordination of services for persons of any age with any disability and their families living in Suffolk County. Referrals, information, education, resources, follow-up and advocacy are important components of this service for more than 2000 individuals and families annually, as well as coordination of residential needs for the Suffolk community.

In-Home Respite Services

Through In-Home Respite Services, a brief respite is provided to the caretakers of an individual with a disability. Since 1986, UCP of Greater Suffolk has provided In-Home Respite Services funded by a grant from the New York State Office of Mental Retardation and Developmental Disabilities (OMRDD). This service currently assists approximately 80 people a month.

Afternoon Respite Services

UCP of Great Suffolk offers afternoon Sports and Art respite services to individuals with disabilities.

The Sports Respite Program assists adolescents (ages 12-21) in understanding teamwork, discipline, rules and regulations of the games, as well as "how-to's" of a particular sport. The program is held on Monday and Wednesday from 3:00 PM until 6:00 PM.

The Art Respite Program serves twelve adult individuals. Through the exploration of the arts, this hands-on program seeks to increase the self awareness and self-esteem of the individuals. They receive personalized training in the arts to allow them to acquire skills and increase their self-confidence. They program is held on Monday and Wednesday from 3:30 PM until 6:00 PM.
