ADULT DAY SERVICES

Associate Director of Adult Day Service (Hauppauge)
Provides daily operational and managerial oversight for the Adult Day Services Programs and Services including core areas of policy and procedures, all accreditation standards, staff training, person centered planning focus, staff performance improvement and development; daily health and safety of program facilities and services, and monitoring of budget implementation. Bachelor’s Degree required, Master’s preferred in health related field and 5-10 years related administrative experience in providing services funded by OMRDD. First Aid and AED certification. Clean and Valid NYS Driver’s License required.

Manager of Service Coordination (Hauppauge)
Provides guidance, support and instruction to Service Coordination staff . Responsibilities include interviewing, hiring and training employees, planning, assigning and directing workload. Must have management and related experience working with developmental and TBI disabilities. BA required, MA preferred. Must have clean and Valid NYS Drivers License.

Day Hab Asst. (Bohemia)
Responsible for the implementation of individualized services in all Day Habilitation areas emphasizing community integration and in-house activities to enhance the People Served in their chosen outcomes affecting his/her capacity to live, work, socialize, and participate within the community. High school diploma or general education degree (GED) required. Valid clean New York State Driver's License Valid First Aide Certification (Responsibility of employee).


RESIDENTIAL

Diet Tech (P/T)
Responsible for consulting with Medical, Nursing, and Residential Staff to ensure that proper and adequate foods are consumed by the individual served. Associate's degree (A. A.) or equivalent in Applied Sciences from two-year college or technical school and one to six months related experience and/or training.

Direct Care Aides (Various locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid drivers license.

Assisant Residence Managers (Patchogue/Stony Brook/Islandia)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Supervises a total of up to 13 employees in the Residence. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training. Supervisory experience in the field of developmental disabilities. Valid clean NYS Driver's License. SCIP-R, CPR, and AMAP certification preferred.

Occupational Therapist (P/T) (Various Locations)
Bachelors Degree required. Graduate of school accredited by A.O.T.A. Must have valid NYS Drivers License.

RNs/LPNs- Full Time Days/Eves and Per Diem (Various Locations)
Must have valid NYS Drivers License


EDUCATION
 

HEALTH CENTER
Physical Therapist (Central Islip)
Treatment of patients through the use of physical agents, appropriate therapeutic exercise, stretching, joint mobilization or other therapeutic techniques. Bachelor’s Degree required and Graduate of school accredited by A.P.T.A. NY State License in Physical Therapy. Valid and clean NY State Drivers License required.

Physical Therapy Asst. - (Central Islip)
Treatment of patients through the use of physical agents, appropriate therapeutic exercise, stretching, joint mobilization and other therapeutic techniques. Data collection to assist physical therapist in evaluation of all patients referred for physical therapy services. Graduate of school accredited by A.P.T.A. and NY State Certificate in PTA. Must have means and ability to travel between Agency buildings.

Adult Services Assistant - P/T - (Central Islip)
Assists in patient care under the supervision of therapists. Reception of Rehab patients, clean up of therapy rooms, set-up equipment and maintain supply inventory as well as additional clerical duties. HS Diploma or GED required. Knowledge of basic office procedures and computer literacy helpful.
