bullet ADULT DAY SERVICES

Service Coordinator (Hauppauge)
Provides information, linkage, referral, and monitoring of services; advocacy to empower individuals and families; and help to advance the independence, productivity, and full citizenship of people with developmental disabilities. Minimum of Associate’s degree (A.A.) or equivalent from two-year college or technical school in Health or Human Service Field and one year’s experience with people with developmental disabilities (unless Master’s degree, for which experience is waived). Valid and Clean NYS Drivers License required.

Program Health Assistant (Hauppauge)
Observe program participants and communicate any health issues to RN Supervisor carry out daily assignments of health treatments and monitoring; carry out program duties to assure health, safety and active treatment program participants. High school diploma or general education degree (GED) and one to three years related experience and/or training or equivalent combination of education and experience. Current First Aid Certification, CPR and AMAP preferred.

Teacher Aides Subs - (Commack)
Assists classroom teacher in curriculum implementation by working with individuals or groups of children as assigned by classroom teacher. Assists in implementing strategies suggested by clinicians to the teacher. Performs frequent daily lifting of individuals with disabilities (ages 2-21), for their activities of daily living needs. High school diploma or general education degree (GED) required.

Physical Therapist F/T
Responsible for the evaluation and treatment of those consumers who have been referred by a physician, and assigned by the Director of Educational Services. The consumer referred may have difficulties of orthopedic, neuro-muscular, and/or developmental origin. Bachelor's degree (B. A.) from four-year college or university. Graduate of a school accredited by APTA. Two years "pediatric/rehabilitation" experience necessary. NYS License or eligibility upon graduation in Physical Therapy required.

Residence Managers (Various locations)
Manages one subordinate supervisor (Assistant Residence Manager) who supervises a total of up to 13 employees in the Residence.  Responsible for the overall direction, coordination, and evaluation of these units. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to three years related experience and/or training.  Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required.  SCIP-R, CPR, and AMAP certification preferred.

Assistant Residence Managers (Various locations)
Perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities.  Function as Manager in Manager's absence. Supervises a total of up to 13 employees in the Residence.   Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. High school diploma or general education degree (GED) and one to six months related experience and/or training.  Supervisory experience in the field of developmental disabilities necessary. Valid clean NYS Driver's License required.  SCIP-R, CPR, and AMAP certification preferred.

Registered Nurse (Per Diem)
Supervise and train direct care staff regarding medication administration, universal precautions, infection control, health and safety issues, etc. Implement guidance policy as appropriate to address medication errors or violations of health and safety procedures by direct care staff. Insure maintenance and review of all medical records per state and federal guidelines and UCP policy. Complete yearly recertification, including practicum, for each medication certified person health and safety issues, etc. Available by phone 24-hours to address medical emergencies. Associate's degree (A. A.) or equivalent from two-year college or technical school and one to three years related experience and/or training. New York State Licensed Registered Nurse.

Direct Support Professionals- F/T, P/T & Per Diem (Various Locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid NYS Driver's License.

Physical Therapist (P/T Contractor)
Bachelor's degree (B. A.) from four-year college or university. One to three years of experience necessary. Graduate of a school accredited by APTA. New York State License in Physical Therapy required. Experience with OMRDD population a plus. Flexible schedule (8 hours/week).

bullet HEALTH CENTER

Assistant Director (Central Islip)
Responsible for managing operations of large comprehensive Article 28 & 16 Health Clinic. Oversee daily operations, implement NextGen electronic health record, provider training, and ensure efficient compliance and incident review procedures. Must have 4 year degree in healthcare/business related field. Minimum 5 years experience required.

Psychiatrist (P/T Contractor) Central Islip and Port Jefferson
Diagnose, treat and evaluate mental health issues. Adult and pediatric experience preferred. Must be Board Certified or Board Eligible in psychiatry and NYS licensed. Experience with OPWDD population preferred.

Medical Assistant (Central Islip ad Port Jefferson)
Assists in examination and treatment of individuals served under direction of Physician. One year or more MA experience and must have Phlebotomy skills and CPR training. High school diploma or general education degree GED required. National Certified Medical Assistant by National Center for Competency Testing and Certified Phlebotomist Technologist by National Phlebotomy Association necessary. Valid and Clean NYS Drivers license required and vehicle for transportation to other sites.
