ADULT DAY SERVICES
Vocational Rehab. Admin. Mgr
Responsible for the Coordination of all VR information measurement & management for performance improvement.  Oversee vending program business logistics.  Responsible for keeping current with all VR Policy and Procedures, filing and inventory systems.  College degree required, bachelors preferred.  Strong organizational and customer service skills necessary.  Experience working with management teams.  Strong computer skills including Microsoft Word, Excel, Data Bases (Access preferred).  ACT! and Desktop Publishing a plus.

Vocational Rehab. Job Coach  (Hauppauge)
Responsible for all job coaching service delivery to individuals with disabilities in all employment models. Liaisons with employers and co-workers for person served objectives.  High school diploma or general education degree (GED); or one to three months' related experience and/or training; or equivalent combination of education and experience.  Valid NYS Drivers License required.

Co-Site Manager
Management position with co- responsibility for the day-to-day administration and implementation of all operational and Day Habilitation components under the required Governmental, Agency and CARF Regulations. This is a hands-on position Bachelor's degree (B. A.) from four-year college or university preferred with a minimum of 5 years experience with individuals with disabilities.

Program Aides
Responsible for the daily caring of individual's needs, as related to all personal hygiene, feeding and toileting, repositioning, ambulating, and other goals prescribed by consultants. High School diploma or GED required.


RESIDENTIAL

Assistant Residence Managers (Stony Brook)
Assist Residence Manager to perform all duties related to oversight of assigned residential program serving individuals with developmental disabilities. Function as Manager in Manager's absence. Minimum of one-year experience working with individuals with developmental disabilities. High school degree or GED required. Must have a clean and valid NY State Drivers License.

Recreation Therapists (ICF-6)
Responsible to structure, incorporate and monitor meaningful individualized group activities for persons with developmental disabilities. Bachelor’s degree plus six months experience required. Must have a clean and valid NY State Drivers License.

Communication Specialist (ICF7)
Plan, evaluate, and structure a communication program as needed into each clients daily activity schedule including treatment goals and objectives that will be developed, monitored, and updated as needed. Keeps information current and representative of resident’s present functioning and developmental level including: Annual evaluation of the client’s strengths and needs; Annual evaluation of the client’s level of functioning in reference to communication skills, Annual assessments; Quarterly review; Monthly summaries; Monitor data sheets in reference to communication goals and objectives; Insure that all goals and records meet with State and Federal regulations. Evaluate, help achieve treatments, goals and implement individual communication programs if needed in conjunction with the other disciplines. Bachelor's degree (B. A.) from four-year college or university and one to six months related experience and/or training. Must have a clean and valid NY State Drivers License.

Applied Behavior Specialists - Part Time (Commack)
Develops and maintains goal plans related specifically towards addressing behaviors of individuals served under supervision of a licensed Psychologist. Responsible for evaluations and testing of individuals served and training staff members. Master's Degree plus four to ten years related experience.

Direct Care Aides (Various locations)
Assist individuals with developmental disabilities in activities of daily living. High school degree or GED required. Must have a clean and valid drivers license.

Registered Nurse - Part Time (ICF31)
To provide skilled nursing services to individuals with developmental disabilities within residential services. Must have a current NYS registration.

EDUCATION

Physical Therapist or PTA(Commack)
Provides services to bring about changes which enables an individual to function better in their daily life and to move more efficiency. This includes the range of motion of the joints of the body, the strength of muscles, posture, control of movement, balance, and coordination. Masters degree required.

Substitute Teachers (Commack)
Responsible for the education of each student on his/her caseload in all major developmental domains. This will require that considerable emphasis be placed on coordination with other disciplines involved with the students program. Provides appropriate individualized instruction to meet the needs of students according to their Individualized Education Plan. Minimum, Bachelor's degree (B. A.), preferably a Master of Science, required from four-year college or university. Previous teaching experience is preferred. NYS Teaching Certification in Special Education.

Full Time & Substitute Teacher Aides (Commack)
Provides instructional support for classroom and reinforcement to children under the direction and guidance of teachers. Works with students individually or in small groups. High school degree required.

We are also presently seeking Teacher Aides for our 6 week summer program to begin on July 2nd.


HEALTH SERVICES

Medical Collector - P/T (Commack)
Must have knowledge in Medicare/Medicaid & third party collections and be able to follow-up with all insurances, A/R & claims denial resolutions.  Medical Mgr a plus.  Must have High School diploma or equivalent. 


OTHER
Service Coordinators
Provides information, linkage, referral, and monitoring of services; advocacy to empower individuals and families; and help to advance the independence, productivity, and full citizenship of people with developmental disabilities. Minimum of Associate’s degree (A.A.) or equivalent from two-year college or technical school in Health or Human Service Field and one year’s experience with people with developmental disabilities (unless Master’s degree, for which experience is waived).  Valid NYS Drivers License required.

Maintenance Mechanic (Commack)
Maintain a clean working environment for all staff and visitors. Ensure all areas are safe. High school diploma or general education degree (GED); One to three months’ related experience and/or training. Valid current New York State Driver License.
